a. CS Form 212 Personal Data Sheet (2017 Revised Form) must be properly accomplished and notarized;
b. Copy of Transcript of Records & Diploma;
c. Certification of work experience;
d. Copy of appropriate eligibility;
e. Last performance rating from current/previous employer; and
f. Copy of related training/seminar certificates.
Application documents may either be mailed via courier or dropped physically at the ๐๐๐๐ ๐๐๐๐ข๐๐, ๐๐ง๐ ๐
๐ฅ๐จ๐จ๐ซ, ๐๐๐ฆ๐ข๐ง๐ข๐ฌ๐ญ๐ซ๐๐ญ๐ข๐จ๐ง ๐๐ฅ๐๐ ., ๐๐๐-๐๐๐ข๐ง, ๐๐ข๐ง๐ ๐๐ฒ๐๐ง, ๐๐๐ง๐ ๐๐ฌ๐ข๐ง๐๐ง. Submission of application may also be done electronically through the ๐๐๐-๐๐๐๐ ๐๐จ๐ ๐๐จ๐ซ๐ญ๐๐ฅ.
