a. Personal Data Sheet (2017 Revised Form) MUST be properly accomplished and notarized;
b. CS Form 212 (Personal Data Sheet AND Work Experience Sheet);
c. Copy of Transcript of Records & Diploma;
d. Certification of work experience/s;
e. Copy of appropriate eligibility;
f. Last performance rating from current/previous employer;
g. Copy of related training/seminar certificates; and
h. Potential Assessment Form (to be signed by immediate supervisor) for government employees only.
Application documents may either be mailed via courier or dropped physically at the HRMD Office, PSU- Main Office in Lingayen, Pangasinan. Submission of application may also be done electronically thru the ONLINE JOB PORTAL via the HRMDO’s HRIS Job Portal.
